- Support the implementation of SAMS health programs including Type 1 Diabetes project activities in accordance with project plans and timelines.
- Coordinate the identification, registration, and follow-up of beneficiaries eligible for project services.
- Distribute glucometers, test strips, and other diabetes-related supplies to beneficiaries according to approved procedures.
- Provide beneficiaries and caregivers with basic education on glucometer use, diabetes self-management, and project services.
- Maintain regular communication with beneficiaries to ensure continuity of care and adherence to project requirements.
- Coordinate and maintain communication with community-based organizations (CBOs), health facilities, and other relevant stakeholders to facilitate project implementation.
- Support outreach activities and referrals to ensure eligible beneficiaries are enrolled and receive needed services.
- Conduct field visits as required to monitor project activities and strengthen stakeholder engagement.
- Accurately collect, enter, update, and verify beneficiary and project data in designated databases and tracking systems.
- Maintain organized and confidential beneficiary records and documentation.
- Track project indicators and monitor progress against established targets.
- Prepare regular reports, success stories, and project updates for submission to the direct supervisor.
- Monitor stock levels of glucometers, test strips, and other project supplies.
- Maintain inventory records and support stock management processes.
- Prepare and follow up on purchase requests and procurement-related documentation in coordination with relevant departments.
- Coordinate with operations and logistics teams to ensure timely availability and distribution of project supplies.
- Support other administrative and operational tasks related to project implementation as assigned.
- Ensure all project activities are implemented in accordance with organizational policies, donor requirements, and relevant health standards.
- Handle beneficiary information with a high degree of confidentiality and professionalism.
- Participate in project meetings, trainings, and capacity-building activities as required
- Bachelor’s degree in nursing.
- Minimum of 2–3 years of experience in a nursing or community health projects.
- Familiarity with data entry, electronic filing systems, and office software (e.g, Microsoft Office, Google Workspace).
- Proficiency in data entry and electronic records management.
- Experience in reporting.

General objective of the position:
The Community Health Nurse is responsible for supporting the implementation of SAMS health projects including Type 1 Diabetes Management Project by coordinating activities, managing beneficiary data, and ensuring effective communication with beneficiaries and stakeholders. The role includes distributing glucometers and related supplies, conducting diabetes awareness and education sessions, maintaining accurate records and databases, monitoring beneficiary follow-up, and supporting reporting requirements. The nurse serves as a key focal point between beneficiaries, healthcare providers, community stakeholders, and project staff to ensure smooth project implementation and timely service delivery. Success in this position requires strong organizational, communication, and data management skills, as well as a solid understanding of diabetes care and community health programs.
